To the members of the Prince Hall Family, attached you will find the package explaining the workings of the MWPHGL Life lnsurance Program, along with the application paperwork required to enroll in this plan.
The enrollment runs from December 1, 2011- January 15, 2012 ONLY.
All documents MUST be in the Grand East by that date,
NO EXCEPTIONS.
Please complete the information where indicated as follows:
Page 1: Member Information Form, complete top of page and sign at bottom
Page 2: Guardian Life lnsurance Application, complete and answer
questions 1-5 and sign.
Page 3: MWPHGL Member Agreement, complete all appropriate information and must be notarized.
Page 4: Member And Advisor Agreement must be signed by member and advisor
Page 5: Member Beneficiary Agreement
Return ALL pages and your deposit in the amount of $150.00 (annual)
or $75.00 (semi-annual) payable to:
